When I set the pinmode on the LED/output pin to on or off then they work as expected, (being high/low) but it doesn't change when rumble is sent from the game.

so I think it's more to do with how I get the computer to recognize the Leonardo as a vibration capable gamepad. I have tried with LED_builtin instead of using the actual output pin and got nothing.
